Marquis Health Consulting Services introduced a comprehensive new chronic disease support model last week. The platform, which was immediately adopted by 10 Marquis client facilities, integrates palliative care, pastoral support and other services with specialized clinical programming.
“The Marquis Chronic Disease Support Program emphasizes the physical and spiritual well-being of patients with chronic diseases – it is a next-level model of care for (skilled nursing facilities ),” Jennifer Hertzog, vice president of marketing and business development, mid-Atlantic, for the Brick-based retirement home consulting firm, said.
Marquis’ patient-centered chronic disease supportive care model is led in each facility by leading regional specialists. It incorporates palliative care, which is administered by a trained team of doctors, nurses and specialists and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The aim is to improve the quality of life of patients and their families.
The program developed by Marquis also incorporates disease-specific health education and coaching, multi-specialty and coordinated care, full-time respiratory therapy, support from social services, and links to community resources and enrichment activities. of life. It integrates with a range of specialty programs – such as cardiopulmonary rehabilitation, chronic kidney disease management, dementia care and others – offered at Marquis client facilities.
